CLINIC CARDS
Also included in each Back-To-School Packet is a form printed with two (2) clinic cards. Please complete both cards in their entirety and return on or prior to the first day of school. DO NOT SEPARATE. One copy will be maintained in the clinic and one copy will be kept with grade level team leaders.In the event of an illness, injury or other emergency involving a student, a FSUS clinic staff member or other authorized school employee will attempt to contact the primary parent and speak in person to the parent or guardian at the phone numbers listed on the clinic card.
If there is no answer, we will continue down the list of phone numbers until we are able to discuss in person the students illness, injury or emergency. If we are unable to make contact we will return back to the primary parent number and leave a message if possible. Additional space is provided on the cards for any special physical conditions/allergies/instructions.
