IMMUNIZATIONS AND HEALTH REQUIREMENTS
Kindergarten - 12th Grade Requirements
All students must have the following:
1. Birth Certificate.
2. Physical examination form dated within 12 months prior to school starting. Physical findings must
be documented on the form HD 3040,which is available at the physician's office or the Health
Department.
3. Immunization records must be documented on a Florida certificate of immunization, form HD 680,
which is available at the physicians office or the Health Department. Kindergarten - 12th grade
students are required by Florida law to have the following immunizations. Kindergarten - 5th grade and
7th grade require additional immunizations, see below.
- Five doses of D.P.T. vaccine (Diphtheria, Pertusis and Tetanus). If the forth dose was administered on or after the fourth birthday a fifth dose is not required.
- Four doses of Polio vaccine (OPV or IPV). If the third dose was administered on or after the fourth birthday a fourth dose is not required.
- Two doses of MMR vaccine (Measles, Mumps, and Rubella).
- Completed Hepatitis B series, three shots within a 6-month period. Some physicians are using a two dose series - this must be documented.
- 1st - 7th grade students are required to have one VZV vaccine (Varicella/Chicken Pox) or documentation of the disease.
- Kindergarten students are required to have VZV vaccine (varicella/chicken pox) or documentation of the disease.
- 7th grade students are required to have the TD booster (Tetanus-Diphtheria).
